Figuring out BASAL and BOLUS is NOT hard, it does take time and patience. First you MUST understand the roll of BASAL & BOLUS.
BASAL..just like it sounds, provides the BASIC insulin needs for you body 24/7 with NO food & NO exercise. BOLUS provides what insulin you need for FOOD, and also used to make correctins for high BS if needed.
Like I said it does take time and patience, ALWAYS play it safe and make any changes in SMALL increments and TEST often.
HOW I did it...........
I took my morning Lantus (I am on a split dose twice a day). Then I tested every 30 minutes or so to see if my BS level was climbing or dropping. WHEN in doubt test more often. Once I got my BASAL (Lantus adjusted, my BS level was quite stable for at least 5hrs. REMEMBER NO FOOD< NO EXERCISE, I watched TV and read.
I like my BASE BS level at 100, you and your doc may want it a little higher.
RULE....... for adjusting BASAL,
START with your CURRENT BASAL insulin dose!
IF your blood sugar INCREASES as lunch time approaches then you need to INCREASE you BASAL.
LIKEWISE..if your blood sugar DECREASES as lunch time approaches then you need to DECREASE your BASAL.
Lets say you start your BASAL adjust ment on day 1 and make a small adjustment, you need to let your body acclimate to the new basal level a couple days before making another adjustment. So you do not want to make another adjustment till at least day 3, maybe day 4. I made my adjustment in increments of 1 or 2 units at a time, any more than that could be risky! My BASAL rdose is 11 units, twice a day or a total of 22 units of Lantus per day.
IMPORTANT.......you can NOT figure your BOLUS untill AFTER you have established a good STABLE BASAL.
When I did mine my system was in effect in total shutdown and my carb ratio then came out to be 1:10., not my system is functioning a little so I usually use a 1:12 to 1:15 or 1 unit of Humalog per 12 to 15 grams of carbs.
BEFORE I got my doses PROPERLY figured out I was a disaster! They ahd me on 25 units of Lantus 2x day so I was bouncing all over the map. They started me with a 1:15 or 15 grams of carbs.When I found my PROPER carb ratio my bolus was in almost lock step with my food and my BS would be back close to where it was before the previous meal.

MAKE sure you understand the process BEFORE trying to do it!
