I have found in a number of books, from a number of people (CDE , Endo )
and here in many forums this rule of thumb:
your daily insulin use should be approximately 50% Basal & 50% Bolus.
Why?...
Is there some other correlation to the 50 /50 rule & control I should be aware of?


I have been playing with the ratios over the last 2 months and have finally gotten some good control and the ratio is more like 70% Basal & 30% Bolus.

I got you beat :) because of diet and exercise I will probably not take any bolus today at all, just 18u of basal. For the past two weeks I have been taking 18u basal and just 2u of bolus and those two where at breakfast.

I think I am still honeymooning, but I have been only eating about 60 - 70 grams of carbs a day, with half of those at breakfast.
My doctor has me on 50/50.
I probably do between 50basal/50bolus and 75basal/25bolus. It depends on the day, what my BG's are doing, and what I'm eating. Most days it's probably like 60basal/40bolus.

I find that the ratio mostly depends of how many carbs you eat! Someone who eats a medium to high carb diet (the one ADA recommend) should have around 50% - 50%.

If you eat low carb, then of course you bolus less!! and therefore a 70 -30 or other ratio could be much more normal.

I don't think this ratio can tell us how good your control is... depends on too many other factors. We have enough numbers to worry about-- I decided that I don't care about this one!
Effectiveness is the first measurement of truth - in other words, do what works!
I think the 50/50 is sort of a general guideline - nobody conforms to a standard definition.
Everyone is different and I think the 50/50 rule is a place to start whent he doctor is guessing at where to start the insulin dose. From there is needs to be adjusted according to BG, activity level, carb intake, and many unknown factors as well. I use about 30% basal with 70% bolus. Most of my bolus is for food not correction and I eat approxamately 110-140 grams of carbs per day. I am vegitarian so I use a few more carbs than those who eat a lot of meat.
