I am on a liquid diet in preparation for lap band surgery. I am supposed to have 2 protein shakes per day. I will be on this diet for at least 9 more days.

I make the shakes with

1 scoop of protein powder (8g)
8 oz of low fat milk (11g)
1 small plain, nonfat yogurt (6g)
1/2 packet Splenda (1g)

Adding the carbs, I get 27g. That's what I bolused for the first time. Two hours later with no eating after the shake, my blood glucose was close to 400. I have, through trial & error found that I need to boluse for 100g of carbs.

Any ideas on what is going on?

I would be careful about trusting the stated carb count of a protein powder. Sometimes they "lie" and it contains added sugars that they don't count. Look carefully through the ingredients.

Second, remember that you may need to bolus for protein. I consider that half the grams of protein are converted to glucose, so I bolus for half the protein as carbs.

And finally, where is the fat. You absolutely need fat. You should be eating full fat yogurt and milk and even adding fat. Consuming a high protein diet with low/no fat can result in protein poisoning (also called Rabbit Starvation). You need to be careful, perhaps you could add coconut oil to your shake?
