Are you, or do you know someone who is over 18 years old, has type 2 diabetes and takes medication to treat their diabetes (it does not matter what kind). I am conducting a study for my schooling and need more participants.

The study has been reviewed by a board of ethics at AT Still University. It was granted an exemption because it is anonymous.

There are no monetary payments or gifts associated with the study. Your results are entirely anonymous and will remain confidential. The entire survey will take less than 10 minutes of your time and it is a one time survey.

PLEASE PASS THIS ON TO YOUR FRIENDS AND RELATIVES, EVEN IF YOU DO NOT THINK THEY HAVE DIABETES, THEY MAY KNOW SOMEONE!
