It happened to me and others as well:

I had the buttons pop off once, when I dropped the G4, but snapped them back into place. I'm now using the Tallygear case, and no further problems (keeping fingers, eyes, and knees crossed!)
The center button and "donut" ring fell off my first G4 too.
I got my third receiver a couple of months ago. So far, so good.
It happened to me after about 5 months as well.
Same here, even with the Tallygear case to protect it. First one after 1 month and only 2 months later it happened with the new one. There was no problem to replace it.
Mine fell out about a month ago. It looked like one of the two tabs that hold the donut ring in snapped. Fortunately, Dexcom was very willing to replace it.
Looking ahead for when it happens to me, is it possible to glue these back on?
I just placed a piece of scotch tape over the button and donut ring, and it works fine.
The same thing happened to my first receiver, only a couple of weeks after I got it back at the end of last year. Thankfully nothing has happened to the replacement so far but have to admit I am very careful to avoid it happening again (if at all possible).
I found mine in three pieces laying in bed next to me singing away during a very sweaty , very low hypo experience. Dexcom replaced it and shipped overnight and the Sensor I had to replace. and yes it looks like tabs were broken off. Sure don't know how that happened.It was about 8 months old and was my second G4
The first was replaced because the display had a large white line running down the right side of the display window.
