Don't know if this has been discussed amidst the pod failure discussions, but thought I would throw it out there since I haven't read any other discussions about it.
I have noticed that my new pods seem to be making the sound associated with the ejection of the canulet at different times. Every once in a while, I will just hear (and feel) it when it is on my body and the initial ejection has already happened. This just happened on the pod that I put on, and I must admit that the initial ejection felt very light. 15 minutes later it made the ejection sound again. I also hear it during the priming process. Could the canulet be misfiring and causing all of the pod failures?

There is something different about the priming process of the new pods, and there is a firing-type noise that is heard during priming. Maybe some time of pre-release of the firing mechanism? At the end of priming, check your pod to make sure the cannula hasn't actually fired. I've never had that happen, however.
What ever noises you are hearing they will not me a re firing of the insertion mechanism. Just took one of the new ones apart and it is totally different. A crank shaft, a connecting rod and a heavy coil spring. Once it triggers there is no way that coil spring can reset. Interesting concept but looks like potential for mechanical problems that the old ones didn't have. Do check the cannula after the initial insertion. Good luck.
Is it possible that it gets stuck somehow and does not completely fire?
I have had this happen a couple times. During the insertion process, I don't feel anything and can't see the canula. I left the pod on and waited for a bit because I read that this could happen, and about 30 seconds to a minute later, heard the click and felt the insertion. It kind of made me jump a bit because I wasn't expecting it.
I've experiences this too. And while filling the pod last time, I could have sworn that the canula fired. I was ready to trash a perfectly good pod, but after removing the cover it was not ejected and the pod is working as normal now.
I'm one of those that takes things apart, so I know a little about the inner workings of the pods. There's basically only one motor in the device and it is used to do everything, literally, everything. Including both insulin delivery and canula firing. So I'm guessing that the new pods have a little smaller mechanism that is a little more loud at times that makes it sound as if the trigger was pulled on a canula, but must be some other gear or something getting bound up and clicking.
Oh well, as long as they work. And so far they've worked quite well for me.
On other posts, people have said that they have experienced 'delayed deployment' of the canula, but the pod did function normally. Some have just 'tapped the pod' and snap! the insertion. I have had (1) delayed deployment, followed by normal function. It's always something, yet I love the Pod.
