Overnight highs? What do you think is going on?

Hi people,

I need some advice, started a pump a while back, it's going great sugars are staying between 100-160 during the morning, afternoon and early evening which is making me happier than a mouse in Faustman's lab...

But, I go to bed around between 10-12pm, I also check before I go to be and my levels are always around 100..

But I keep waking up around 4/5 am feeling awful, I check my BG and it is always the same.. 278+!

So..

I have taken the following action,, sugars are going high over night, so I gradually increase basal rates between 1-6am 10% each night hoping after a few days they will go down, did this for 4 nights, not a spot of difference, they even increased abit even though I had markedly increased my rates?

Right so back to the drawing board!

Perhaps I am going low, e.g. going to bed around 100, I am staying low during the night and my liver/symogi effect is kicking in and is chucking out some glucose following a protracted low and causing this high...
So I dial back the basal significantly, check at 2am, 3am etc, levels are fine no hypo.. Wake up between at 4.5am always after disturbed, weird dreams with my sugars at 275+, literally every night always the same?

So now what do I do ha?

It's such a shame as I work so hard when I am awake keeping within my target rates and this feels like a kick in the teeth, as I know I am spending 3-4 of my 24 hours with my sugars far too high :(

Any experience of this, any thoughts as to what may be going on???

Perhaps you have the dreaded Darn Phenomenon (DP). Many of us suffer from DP. I believe DP occurs as our body naturally clears insulin and generates higher blood sugars to wake up. The so-called Somogyi effect is actually thought to be quite rare. I require higher basal insulin levels overnight (like more the double daytime levels). The best way to actually see is to set your alarm for the early morning hours (3-5 am) and see what your blood sugar is doing. My bet is that a 10% increase is not nearly enough.

Yes I think it maybe a case of DP....

I was reluctant to really ramp up my basals for fear of a night time low... But I think it is looking like I will have to do so..

Hi there. It sounds like you may have the culprit figured out. I'm dealing with an 8 year old, so the rules are probably a little different, but I find night-time can be a varied affair. Caleb's basal rates are the highest between 8 and 11pm, and this is also the period that varies the most. I think growth plays a big part in that. But I also think what he eats for dinner affects his overnights. If he has a lighter carb meal, I know I have to be a little more conservative with his basals. I know this should all be taken care of with his meal bolus, but the practical application has been easier using his basals. The other thing with Caleb that seems to be unusual is that his correction factor is the highest between 10pm and 3am. His doctors always raise an eyebrow at this, but it is decisively the case. His basals are the lowest between midnight and 5am though. Anyhoo...this is all rather unrelated to your issue except that I totally understand the reluctance to be aggressive overnight, but it is what it is and it sounds like you have the information you need to "ramp it up" a bit to address those highs. I remember when Caleb first started pumping the 8 to midnight period being a complete horror show - uncorrecting 300s night after night and his CDE changed his rates so miserably slowly - it took a while to get him in range and wish we would have addressed it more aggressively.

Hi BSC I also have the drearded DP at 9pm i take a reading bg is 89 107 but by 6am 220's somtimes higher i take 3 pills Janumet actos and glyburide plus 10 units of lantus notghing changes after launch and the rest of the day my bg is fine i think maybe i sleep eat and are unaware of it
